Cancellation Policy

Membership Commitment The House Plan is a monthly membership designed to provide high-touch, functional medicine care. Memberships are billed automatically every 30 days from your original sign-up date. Your benefits renew monthly and do not roll over. Add-On Services Consultations for peptides, hormones, weight loss, detox, or advanced lab testing are available as add-ons. Founding members receive 20% off all add-on services. Cancellation Policy Membership cancellations must be submitted in writing at least 30 days prior to your next billing date. Requests must be emailed to: info@houseofhealthaz.com. No refunds will be provided for partial months or unused services. Once canceled, access to membership benefits will remain active until the end of your current billing cycle. Rejoining After Cancellation If you cancel and wish to rejoin later, your membership will be subject to the current monthly rate at that time and will not be eligible for reactivation at the founding member rate. No Insurance Billing The House Plan is a cash-pay service. We do not bill or accept insurance. Superbills are not provided. Scope of Practice All medical services are delivered by a board-certified Nurse Practitioner within the scope of licensure and state regulations. This is not a substitute for emergency care.
